Cherokee Nation Health Services (CNHS) will be assuming operations at the Claremore Outpatient & Emergency Health Center on October 1st, 2025. We are committed to ensuring a seamless transition of care for patients. To support this, we have published the FAQ and information below.

Cherokee Nation has signed into law the Cherokee Nation Claremore Outpatient and Emergency Health Center Act of 2025, which authorizes $255.5 million to construct a new Claremore health facility. This plan includes $11 million in upgrades to the current facility so that CNHS can bring our standard of high-quality care to Claremore patients. The future is bright for Cherokee Nation Health Services at Claremore!

I have a question about my individual care that is not included in this FAQ. Who should I contact?

CNHS has Nurse Case Managers that are focused on assisting CIH patients throughout this transition process. Contact information for each department is listed below:

Will CIH patients need to complete CNHS patient registration paperwork?

To ensure that CNHS has all relevant information, patients will be asked to complete registration forms, present insurance forms and eligibility documents, and sign other required paperwork during their first visit after October 1st.  

Prior to October 1st, patients can complete a medical records release form to ensure their records are transferred from CIH to CNHS. Submitting this form will also allow CNHS to create a patient chart within our system. Forms can be completed with the Registration team onsite at Claremore or at any CNHS location.

Who is eligible for services within CNHS?

CNHS provides services to all registered members of federally-recognized tribes. Additionally, mothers carrying future tribal citizens are eligible to receive prenatal and OBGYN care through CNHS.

Will I need coverage to be eligible for services?

While coverage is not a requirement, we encourage all patients to meet with a Patient Benefit Coordinator (PBC) to find out if they are eligible for SoonerCare, or another form of coverage. To schedule a meeting with a PBC, please call 844-749-4263.

What information do expecting mothers who receive care at CIH need to know?

To discuss questions for your individual circumstances, please contact the Nurse Case Manager, Katie Eubanks, at 539-234-3352. 

Full obstetrical care will continue at Claremore Indian Hospital through September 15, 2025, so patients due before September 15th can deliver at CIH; patients due on or after September 15th will need to deliver elsewhere. To discuss available options, please contact the Nurse Case Manager, Katie Eubanks, at the number listed above.

Will there be any changes for Purchased and Referred Care (Contract Health)?

Our patients should not expect to notice any changes regarding Purchased and Referred Care. Our goal is to process referrals faster than they have been in the past for this location.

Will patients continue to receive Primary Care at Claremore?
